At a small town bar, Jake has a beer with Sheriff Rivers who reveals that he discovered Mrs. Warden was one of Jake's patients. The sheriff is angry Jake didn't tell him about it. Jake tells him she was having an affair, but he didn't know who with. While working with Timmy, one of Jake's neurotic patients interrupts. When Jake turns his back, Timmy grabs a sharp tool and cuts himself with it as he's mutilating playing cards. These cards seem to be specifically chosen and Jake studies them later on, by himself. Sylvie is mad that Jake turned his back on Timmy, but she still brings him back for therapy the next day.
